moderators could add a message giving a reason why they edited a message.

Ow, that is broken for over a year now. The plugin was made for MyBB 1.4 and it was just lucky that it did work on some old 1.6 versions. Around 1.6.2 it broke though.
Also it didn't update the last edited field in the db properly and did not apply moderated attachments and parsing of bb code -> if you changed some formatting in the post the post would get broken.
